Registration & Forms

To receive a discount for lessons, fill out an instructor's form : Sylvia"s PACKAGE FORM, Talya's PACKAGE FORM, and submit or contact your instructor directly. For first time students, also fill out the following forms: RELEASE FORM, STUDENT CONTRACT, PHOTO RELEASE, FARM RULES, LEASE AGREEMENT (if applicable). Print and bring the forms with you to your first lesson.

Register for horse camp HERE. After you receive a confirmation email with a QuickBooks invoice, fill out the following forms: RELEASE FORM, PHOTO RELEASE, FARM RULES. Print and bring them with you to camp. 

Please print and sign a RELEASE FORM, review the FARM RULES, and print and sign our BOARDING CONTRACT.

Please print and sign a RELEASE FORM and FARM RULES for each family member/guest/friend who will be visiting the farm.
